The SRS Awards, created by Caisse Desjardins du Réseau de la santé, are designed to highlight and recognize professionals for their commitment to improving public healthcare.

Consent Form
Each person who participated in the project and/or appears in the photo must complete a consent form.
You have two options:
If you fill out the PDF consent form, please send it to us along with your participation form.
Forms filled out online will be sent directly to us.
Picture



The picture must be of good quality for printing. A picture taken with a phone is perfectly acceptable. Although it is preferable to show your team, the image can represent your project.
It must also meet the following criteria:
- – Landscape format is recommended.
- – Choose a group photo rather than several individual photos.
- – Avoid taking a photo of a photo.
- – Screenshots will not be accepted.
- – The image must be clear, in focus, and ideally taken during daylight hours.
- – The photo provided must be the original and must not have been compressed (no images from the web, sent by text message, etc.).
- – No photos in docx or pdf format will be accepted.
- – The file name must not contain any special characters (é, !, ‘, etc.).
- – Visual montages containing logos and text will not be accepted.
